Details of Experience:
Mr. Martinez was admitted to Palmetto Subacute Care Center from Baptist Hospital on October 6th, 2020 after a few days of hospitalization, due to right-sided weakness. Mr. Martinez presents with acute infract L pons, along with thrombus seen in V4 segments of the vertebral arteries and in the basilar artery as per a brain MRI. He also presented with mild basilar occlusion and some aneurism. He was referred to Palmetto for decreased strength, decreased functional mobility requiring increased assistance during bed mobility, transfer and gait, poor endurance to activity, decreased static and dynamic balance.

Upon arrival, Mr. Martinez was greeted by the concierge and nursing teams to give him a warm welcome and ensure he felt comfortable. The rehabilitation team’s first evaluation was made shortly after his arrival and they created a specific treatment plan according to his needs and goals. The most important goal for Mr. Martinez was to be able to walk again using a rolling walker by himself. During his rehabilitation journey, he demonstrated the potential to be able to return to his prior level of function and strong family support that allowed him to achieve his goals and return to his former life. When he was discharged, he was able to walk 75 feet using a rolling walker on a level surface with contact guard assist and minimal assistance in most of his activities of daily living (ADL).

On January 14th, Mr. Martinez was discharged to his assisted living facility. He was very happy and grateful for the dedication and commitment of the team at Palmetto, who wish him all the best!

Details of Experience:
Mrs. Lopez arrived at Palmetto from UM Hospital on October 30th, 2020 with a diagnosis of a closed fracture of the 7th thoracic vertebrae. She resides in a 3rd-floor apartment and was able to ambulate with a rolling walker and perform ADLs with limited assistance.

Upon Mrs. Lopez’s arrival, she was greeted by the Concierge, Marialcira, and provided Palmetto’s beautiful gift bag. Additionally, the Concierge supported the patient to ensure the transition to our community was smooth and pleasurable. Ms. Lopez was also greeted by the nursing team and therapist to ensure she was comfortable, and her needs were met.

At the first evaluation with the rehabilitation department, the team established a treatment plan to improve her functional mobility skills, strength, and balance. Mrs. Lopez required moderate assistance with most of her ADLs. She was only able to ambulate 5 feet with a rolling walker and maximum assistance. She set her goals with the team and started her journey with us.
To facilitate return to her prior level of function, the patient required physical and occupational therapy to promote safety awareness, enhance rehab potential, increase functional activity tolerance, strength and increase her independence with ADLs to improve her quality of life.

After days of hard work with the Rehabilitation Team, Mrs. Lopez was able to increase her capacity to ambulate with a rolling walker to 150 feet and stand by assistance, and safely performed her ADLs from moderate assistance to stand by assistance. She had consistent progress towards reaching all the goals established by her and our team, anticipating her return safely to her home environment.
Mrs. Lopez was discharged on December 23rd. She was very happy and grateful to return to her home in time to enjoy holidays with her family…completely recovered.

Ms. Gomez arrived at our community for rehabilitation after a short stay at Westchester hospital. Ms. Gomez was recommended to seek inpatient skilled nursing and rehabilitation services to build strength, endurance, and support her continuing recovery.

Ms. Gomez was welcomed by our community’s Concierge Services Department and evaluated by our highly-trained nursing and therapy teams. After her initial clinical evaluations, Ms. Gomez was guided through a comprehensive care plan to support her goals, which included support in ambulating, building muscle strength, especially in her lower extremities, and to support her diagnosis of dysphagia through the help of our speech-language pathology team.

Our skilled speech therapy team implemented custom treatment interventions to begin building muscular strength and swallowing techniques. Multisensory stimulation, therapeutic exercises, a diet texture analysis with training in the use of compensatory swallowing strategies, speech exercises, and training in compensatory strategies were provided to ensure that Ms. Gomez could transition through levels of independence with her speech.

Ms.Gomez began to make noticeable progress and continued to build upon the foundation that the team had established since her admission. Her daughter shared, “Wow! I always knew Palmetto Subacute was an excellent rehabilitation center, but this is great! Your services have been an eye-opening experience for our family. I immensely appreciate all of the extra time you guys have provided for us. Since we are so far away, giving us even 5 minutes to see my mom via FaceTime every day makes all the difference, thank you!”

Our team is thrilled to share that Ms. Gomez continues to live in good health, sharing moments of inspiration to those around her and becoming more and more independent with each and every day.

Following a fracture of the left femur, Ms. Landrian was admitted to South Miami Hospital and then transferred to Palmetto Subacute Care Center on March 6, 2020. She needed skilled care and assistance to be able to return home, and with the assistance of the hospital, Ms. Landrian and her family determined Palmetto would be the perfect fit.

Ms. Landrian was welcomed with open arms to the community. She was promptly escorted to her room and was attended by Palmetto’s skilled nursing staff shortly following arrival. Subsequently, she was evaluated by Palmetto’s Physical and Occupational Therapy teams, Social Worker, and Concierge. She was given a personalized treatment plan based on the objective findings of the nursing and rehabilitation team. This plan focused upon the ultimate goal of discharge following full recovery.

Ms. Landrian received physical therapy five days per week, in order to improve and assess functional abilities. She also received occupational therapy five days a week, in order to improve and assess safety and independence with self-care and functional tasks.

During her stay at Palmetto Subacute Care Center, Ms. Landrian celebrated her 79th birthday. “Words can’t describe how grateful I am for my friends and family here at Palmetto Subacute Care Center.”, she explained. “They are keeping me safe during this COVID-19 pandemic. And despite a pandemic going on, they still managed to make my birthday one of a kind by bringing out all my church girlfriends. I had what they call a ‘window birthday’. Me in the facility, celebrating with my friends, who are on the other side of the window. This brought tears of joy to my eyes. I am also very thankful for the therapists who do exercise with me, and the nurses who take care of me. I thank you guys so much.” She stated, in appreciation for the skilled Palmetto team, who strive to keep her happy and healthy.

Ms. Landrian is still continuing her journey at Palmetto in efforts to one day fully recover and safely return home! The Palmetto team is so proud of her amazing progress!

On January 14th, 2020, Mr. Sanchez arrived at our community to continue his rehabilitation journey. He had been previously admitted to St. Catherine’s Hospital for a number of weeks with difficulty walking due to a left femur fracture. It was then recommended that he admit to a skilled nursing community, where he could be best treated with professional physical and occupational therapists. After canvassing the area and evaluating a number of options, Mr. Sanchez and his sister came to the conclusion that Palmetto Subacute Care Center was the best option for exquisite therapy and nursing options.

Mr. Sanchez was welcomed by our warm, friendly staff and escorted to his room to settle in. Palmetto’s nursing and rehabilitation teams soon visited him to evaluate his physical and occupational therapy needs. Based upon their findings, they developed an individual, personalized plan to help ease his pain, improve functionality and mobility, and improve his quality of life. Mr. Sanchez’s ultimate goal was to ambulate 250 feet while demonstrating proper gait mechanics in order to improve his independence and facility daily living activities.

Since being admitted to Palmetto, Mr. Sanchez has far surpassed his original goal. He has increased his independence, can facilitate functional mobility, has safety awareness, increased coordination, improved balance, increased functional activity tolerance, and a minimized fall risk. The Palmetto team is so proud of his progress, and knows Mr. Sanchez is as well! He is a strong, motivated individual, who is known in the Palmetto community for giving therapy his all, taking initiative, and in his own words, “trusting the process”.

In his time with Palmetto, Mr. Sanchez has expressed his gratitude to our nurses, CNA’s, and therapists, stating, “I’d like to thank you all for everything you guys do for me. To me, you guys are family. From the moment I arrived, I was welcomed with lots of love. At Palmetto Subacute Care Center, I feel loved and cared for. From the therapists, who have a lot of patience and have supported me, the nurses, and CNA’s, who have brought peace to me during this challenging time in my life.”

Mr. Eriksen arrived at Palmetto Subacute Care Center due to damage in his vertebrae after falling at home. Mr. Eriksen was recommended to be placed in a rehabilitation center by the team at Baptist Hospital. The team at the hospital shared a list of local skilled nursing
Center and he ultimately selected Palmetto Subacute Care Center.

Mr.Eriksen was warmly greeted by multiple professionals of the Palmetto team who anticipated his arrival. He was evaluated by both the physical therapy team (PT) and occupational therapy team (OT), and a personalized treatment plan was put in place based on their findings. The team also developed a discharge plan and goal in coordination with his care plan.

When he first arrived at Palmetto Subacute, he was walking 50 feet with moderate assistance and 35% verbal cues. Mr. Eriksen was able to perform with moderate assistance and was able to transfer to his bedside chair using a standard walker with maximal assistance, requiring frequent verbal cues. Mr. Eriksen was motivated and willing to participate with therapy. After eight weeks of intensive therapy, he is currently able to walk 125 feet x 2 with stand by assistance and 10% verbal cues. The team is very impressed by the progress Mr. Erikson has made. His nurse, Emily, expressed, “Mr. Eriksen is always up bright and early, ready for his therapy session. When he isn’t scheduled for therapy, he takes initiative and walks with his walker from his room till the end of the hall and back. He is extremely motivated.”

Mr. Eriksen is known for his great advice. He is always happy to share his knowledge with others because, as he says, “you don’t know the difference you’ll make in someone’s life”. Mr. Eriksen stated, “I’m grateful for all the therapists and nurses at Palmetto Subacute. They are patient with me and always take good care of me. I’m excited to continue this journey alongside the therapists and I’m excited to see my progress.”

The entire team at Palmetto Subacute is excited to see the improvements Mr. Eriksen has made. As always, the mission at Palmetto Subacute is to make a difference and bring joy to the lives of those we serve and the team is honored to be along with Mr. Eriksen for this part of his life’s journey.

Details of Experience:
Mr. Valdivia arrived at Palmetto subacute care center on September 20, 2019, from Kendall Hospital. Mr. Valdivia was admitted to Kendall hospital after suffering a fall. “I was repairing a window and taking offshores after the hurricane. I missed a step and fell from the ladder; I believe I was up 30 ft.” Mr. Valdivia fractured his ankle and dislocated his shoulder. He was hospitalized for three weeks and underwent application of multiplanar external fixation and placement of Integra graft. It was discussed that he would need skilled care in efforts to return home. The hospital assisted Mr. Valdivia and his family with the selection process, electing to transfer to Palmetto for rehabilitation upon hospital discharge.

Mr. Valdivia was immediately welcomed by the director of concierge, unit nurses, therapists’ dietary staff with open arms! The clinical team was incredibly supportive of all his needs. Mr. Valdivia began therapy immediately while also getting to know additional staff members and residents. We were happy to see Mr. Valdivia feel at home.

Mr. Valdivia was evaluated by all therapy teams, with goals in mind to increase standing balance and reduce the risk of falls. These interventions would help to decrease the loss of balance during functional mobility and safely perform all functional transfers. Mr. Valdivia’s therapy treatment plan included therapeutic exercises/activities, neuromuscular reeducation, and gait training therapy.

Mr. Valdivia increased his independence and was able to walk 160 feet with minimal assistance a month after being admitted; he began with 0 feet. During his last evaluation done on January 24th, 2020, it was reported that he increased from 160 ft to 170 ft. Mr. Valdivia stated, “Although it is hard for me to walk right now, I’m very happy with how the therapy staff pushes me beyond my limits every day. They remind me that the only limits in life are the limits you set yourself and that everything is possible.”

Over the next few weeks, Mr. Valdivia began to participate in activities frequently, and enjoys conversing with other residents and being social. Mr.Valdivia is known for always listening to Peruvian music and sharing recipes of amazing Peruvian meals with the staff! Mr. Valdivia expressed his gratitude to the staff and is amazed by our patient care skills. Although Mr. Valdivia is a current patient with us, he and the therapy team are continuing to work towards a safe discharge plan. We cannot wait to see the accomplishments he will make in the near future!

The annual survey in the state of Florida is conducted and overseen by the Agency for Health Care Administration (AHCA) in an unannounced visit that evaluates the center’s commitment to quality and compliance with industry state and federal regulatory guidelines. This year’s survey for Palmetto Subacute Care Center issued zero citations.
